POSITION: Licensed Social Worker

POSITION LOCATION: Lincoln – Care Coordination Department

HOURS: Full-time – 40 hours / week – Four 10-hour days or five 8-hour days

PRIMARY DUTIES:

• Assist in the overall management of HAN’s patients, particularly those with psycho-socio-economic needs
• Assist patients with transitions of care
• Connect with community resources
• Serve as a liaison between patients and other agencies

QUALIFICATIONS:

• State of Maine LSW license required
• Must have excellent customer service skills
• Must be proficient with computers
• Prior care coordination experience preferred
