We are dedicated to the communities we serve, to treating you as a whole person without financial or social discrimination, and to remaining responsive and nimble to your needs and to changing times in health care. This is reflected not only in the growth of our organization over the years, but also in the growth of our programs and services, including primary care, behavioral health, dental services, walk-in care, occupational health, prenatal care, podiatry, care coordination, patient assistance/outreach and enrollment, radiology and substance use treatment.
We at Health Access Network believe whole-heartedly in meeting people where they are at, whether this translates to working with you to manage your health care, providing you with patient education, facilitating connections with other resources for care outside our walls, or helping you with setting goals for the health of you and your family. We are extremely proud to not only greet you at our office doors, but also to have dedicated providers who will meet you at your door when needed.
We certainly recognize that providing you with complete care is an effort that extends beyond our walls. In an ongoing effort to improve access and empower you in managing your care, we have placed information at your fingertips through our online patient portal and our new website. Our new online home is designed to capture the energy, diversity, and reverent spirit of our community and our health center, and will serve as a hub for you when you are not visiting us in the office. We will be working to grow the website and the patient portal in the months ahead and will certainly keep you posted!
