Care Coordination
The Care Coordination team at HAN is composed of three nurses and a social worker. As a team we work closely with the patient’s providers, specialists, family, friends, and any other resources identified as helpful or supportive. The team strives to provide a highly individualized level of support that meets the patient’s needs. The team takes into account the whole picture — medical conditions, behavioral health needs, housing, family, finances, community, providers — to help patients achieve the health goals they have set for themselves.
