POSITION: Family Nurse Practitioner/Physician Assistant

LOCATION: Lincoln, Maine

STATUS: Full-time

EXEMPT/NON-EXEMPT: Exempt

WORK SCHEDULE: 40 hours / wk.

OTHER: Sign-on bonus, relocation reimbursement, student loan reimbursement, fully paid health insurance, and flexible schedule

SUMMARY: Responsible for providing primary care services, including assessing and managing patients in the clinic and other locations (assisted living facilities, patient homes, etc.), within the scope of licensing, training, and credentialing.

RESPONSIBILITIES AND DUTIES:

  • Exercises independent judgement in the assessment, diagnosing, and initiation of medical processes or procedures commensurate with training and experience and as credentialed by Health Access Network (HAN).
  • Performs and documents comprehensive health histories and physical assessments, formulates diagnoses and identifies health problems, and performs, orders, and interprets diagnostic studies, such as imaging and laboratory tests.
  • Provides treatment through the use of medications, managing acute and chronic conditions, and providing preventative care to patients.
  • Consults with colleagues, clinical care teams, and specialists when appropriate, to ensure correct diagnoses and treatment plans.
  • Provides information, education, and support to patients and families regarding disease process and treatment plan, thus facilitating communication between patients/ families and healthcare teams.
  • Manages patients in accordance with established HAN policies and procedures.
  • Follows state regulations, HAN policies and procedures, and standards of care when prescribing and monitoring controlled substances.
  • Participates in on-call and coverage responsibilities.
  • Reports in a timely manner deaths, outbreaks of contagious disease, and other information critical to the patient’s and/or the public’s health as required by legal or governmental authorities.
  • Meets quality standards as determined by the QI committee and/or HAN.
  • May participate in the clinical experience of students, residents, and nurses as appropriate.
  • May provide supervision for advanced practice clinicians, including reviewing and cosigning notes, along with providing prompt feedback with any additional and/or alternative suggestions for care.
  • Participates in group peer and other review activities.
  • Attends and engages in all required meetings and evaluations.
  • Completes all required trainings in a timely manner.
  • Follows policies and procedures, including those of HAN, CMS, and other contracted payers with which the group may contract.
  • Accurately assigns current procedural terminology (CPT) and ICD diagnoses codes and completes all documentation in accordance with HAN procedures and policies.
  • Seeks professional growth opportunities that enhance professional practice through both formal and informal activities.
  • Performs other duties as assigned by supervisor.

QUALIFICATIONS:

  • State of Maine FNP/PA license
  • Current DEA license
  • Minimum of 2-5 years’ experience working in a primary care, walk-in/urgent care, and/or emergency medicine setting
